According to the NVD entry, 1.9.2 is affected and not patched.
Removed outdated patched version for Ruby 1.9.2 and updated notes regarding webrick's gem separation.

Not sure whether 1.9.3 was patched or not. If it was patched in 1.9.3, than we should add ~> 1.9.3 to patched_versions.

Was WEBrick patched in 1.9.3? I don't see any mention in the NEWS file for 1.9.3.
