Update codex to 0.135.0

What's new

Previously failed test:
should send attachments as prompt items failed because the snapshot still expected the removed perCwdExtraUserRoots field, then had a trailing newline mismatch after the content update.

Changed:

• Updated ACP client handling for new Codex types, including service tier normalization.
• Removed use of the old skills extra-roots request field because the updated app-server schema no longer accepts it.
• Added explicit no-op handling for new notifications.
• Updated typed test fixtures for new required schema fields: model service tiers, turn itemsView, thread source/session fields, lifecycle timestamps, and MCP pluginId.
• Updated the affected snapshot to match the new request shape.
